Chronic fatigue syndrome (CFS), also called myalgic encephalomyelitis (ME) or ME/CFS, is a complex, debilitating, long-term medical condition. IRRITABLE BOWEL SYNDROME (IBS)

Irritable bowel syndrome (IBS) is a "disorder of brain-gut interaction" characterized by a group of symptoms that commonly include abdominal pain and or abdominal bloating and changes in the consistency of bowel movements.

SIGN & SYMPTOMS

The symptoms of IBS may vary from person to person. Symptoms tend to come and go over time, and often last for several days or weeks.  Symptoms can become worse, often during times of stress or after eating certain foods.The symptoms of IBS are often relieved by a bowel movement (passing stool)

Common symptoms of IBS include:

•Abdominal pain and cramps

•Abdominal bloating

•Diarrhoea and/or constipation (sometimes these may alternate)

•Excessive flatulence (wind)

•Mucus in the stool.

•Urgent need to go to the toilet

•Feeling of not having completely emptied the bowels after going to the toilet

Other less common symptoms may also be experienced, such as:

•Lack of energy (lethargy)

TYPES 

IBS is classified into four types.These include:

1. IBS-C (constipation-predominant)

The person alternates between constipation and normal stools. Eating may cause abdominal spasms or severe pain.

2. IBS-D (diarrhea-predominant)

The patient suffers from diarrhea soon after getting up or after he/she eats something. Sometimes, the bowel movement may be involuntary, and the person may need to make frequent urgent visits to the toilet.

3. IBS-A or IBS-M (alternating constipation and diarrhea)

This is an erratic form of IBS, which is characterized by at least one abnormal bowel movement in a day that may lead to constipation or diarrhea.

Thus, IBS-A is a mixed bad wherein you have not set pattern of bowel activity. Nearly 25% of such IBS episodes result in hard or lumpy stools, while another 25% are accompanied by runny stools.

4.IBS-U (unclassified)

Patients who meet the diagnostic criteria for IBS but whose bowel habits do not fit into any of the above subtypes.

CAUSES 

Researchers don’t exactly know what causes IBS. They think a combination of factors can lead to IBS, including:

ALOES

In this remedy there is a lot of wind, with very loud and forceful belching or flatulence. There is nausea and indigestion associated with nerves or anxiety, especially when anticipating a stressful event like a job interview or exam.

ARSENICUM ALBUM 

Stomach cramps that are worse after taking cold. This remedy suits people who always feel chilly; so much so that even eating cold food such as ice cream aggravates. There is a great deal of diarrhoea that can come on after drinking whilst moving around makes everything worse. Taking regular sips of a warm drink and lying down to rest bring relief.

ASAFOETIDA 

Asafoetida is also a very effective remedy for IBS. There is a feeling of constriction in all directions, especially if muscular contractions in the intestines and oesophagus seem to be moving in the wrong direction strongly indicates this medicine. The person may have a feeling that a bubble is stuck in the throat, or that a lump is moving up from the stomach. The abdomen feels inflated, but the person finds it hard to pass gas in either direction to get relief. Constipation brings on griping pains. Diarrhoea can be explosive , and the person may even regurgitate food in small amounts. Moreover the person may exhibit a strong emotional or hysterical element this remedy is needed.

BRYONIA ALBA

A useful remedy in IBS that is aggravated after eating bread and where the thirst is increased. The stomach may feel so heavy that it is like a stone is sitting there. Belching relieves the pain and tastes of undigested food. Tight clothing and walking about also worsen the condition.

CARBO VEG

The remedy is well-suited to older people or those suffering from general fatigue with stomach pains that are burning, sore or pressing. There are cramps which feel like the stomach is being contracted with a lot of bloating after eating. Butter, fats & rich foods can cause belching, heartburn and indigestion with weakness or faintness.

CHINA OFFICINALIS

Stomach acidity with a great deal of bloating & bitter or sour belching. The appetite swings from being ravenous to being completely off food, with a feeling of fullness after eating a small amount. Fruit and milk aggravate and the pains are pressing or sore. Digestion in general is very slow.

COLOCYNTHIS

This remedy has terrible stomach cramps that are better for doubling up and may be worse from anger or indignation. The pains are short, sharp & pinching and aggravated after eating, especially fruit. Heat & pressure give some relief to the pains which can extend to the lower back or into the buttocks. It is also a very useful remedy for sciatica and neuralgic pain in general.

LILIUM TIG

When this remedy is indicated, the person may make frequent unsuccessful efforts to move the bowels all day and have sudden diarrhea the following morning. A feeling of a lump in the rectum, worse when standing up, is common. Hemorrhoids may develop. Constricting feelings are often felt in the chest. The person is likely to be worse from excitement and strong emotions, and may tend toward irritability or even rage.

LYCOPODIUM CLAVATUM 

This remedy is well-suited to people with IBS & food intolerances, especially onions, oysters, cabbage and beans. The pains are worse in the evening, typically from 4- 8pm and are relieved by passing wind. The inflamed digestive tract struggles to get enough nutrition from food, so there can be loss of weight with fullness and bloating in the abdomen. The appetite is very changeable and can go from being ravenous to very full after a few mouthfuls.

NATRUM CARB

This remedy is often indicated for mild people who have trouble digesting and assimilating many foods and have to stay on restricted diets. Indigestion, heartburn, and even ulcers may occur if offending foods are eaten. The person often is intolerant of milk, and drinking it or eating dairy products can lead to gas and sputtery diarrhea with an empty feeling in the stomach. The person may have cravings for potatoes and for sweets (and sometimes also milk, but has learned to avoid it). A person who needs this remedy usually makes an effort to be cheerful and considerate, but, when feeling weak and sensitive wants to be alone to rest.

This remedy suits people who burn the candle at both ends, working hard and playing harder. Eventually, this stressful lifestyle takes it’s toll and the digestive system suffers. There are severe stomach cramps and acid reflux that are aggravated by mental exertion. Hiccough, belching, fullness and indigestion are worse after coffee, spicy foods and smoking. There can also be a lot of nausea and feelings of being sea-sick. The remedy is also very useful to treat a hangover.

PHOSPHORUS

In IBS, the patient complains of constipation alternating with diarrhea. Stools are offensive and there is much urging for stools when lying on the left side. Marked weakness after stools. Highly sensitive to external impressions such as light, sound, odors, touch, thunderstorms, etc. Restless, excitable, nervous individuals. Affects the kidneys causing albuminuria and haematuria; patient has profuse, pale, watery urine with intense weakness. There is increased susceptibility to colds amongst children.

PULSATILLA NIG

Pulsatilla is also indicated for IBS. There is painful distension of the abdomen with rumbling and pressure in the abdomen. Tightness in abdomen after a meal. Watery, rumbling stools; no two stools alike.Pulsatilla is more indicated mild yielding persons

SILICEA

Silicea is also considered a good remedy for IBS. -Loss of appetite and disgust for meat and warm food in general. Bloated feeling in the abdomen. Colicky pain in the abdomen with constipation. Stool is hard and comes with great difficulty. On the other hand patient may also develop diarrhea that has an offensive odor. Silica positively influences nutrition as well and hence its one of the prominent remedies used for rickets in children.

There are five main types of kidney stones:

•CALCIUM OXALATE 

The most common type of kidney stone which is created when calcium combines with oxalate in the urine. Inadequate calcium and fluid intake, as well other conditions, may contribute to their formation.

•CALCIUM PHOSPHATE

Calcium phosphate stones are much less common than calcium oxalate stones.

Calcium phosphate kidney stones are caused by abnormalities in the way the urinary system functions /renal tubular acidosis,often occurs simultaneously with calcium oxalate stones.

•URIC ACID

This is another common type of kidney stone. Foods such as organ meats and shellfish have high concentrations of a natural chemical compound known as purines. High purine intake leads to a higher production of monosodium urate, which, under the right conditions, may form stones in the kidneys. The formation of these types of stones tends to run in families.

•STRUVITE

These stones are less common and are caused by infections, such as a urinary tract infection. These stones can grow quickly and become quite large, sometimes with few symptoms or little warning.

 

•CYSTINE 

These stones form in people with a hereditary disorder called cystinuria that causes the kidneys to excrete too much of a specific amino acid.These stones are rare and tend to run in families.

•ARGENTUM NITRICUM 

Argentum Nit used for treating kidney stones, which develops due to kidney congestion.

Dull backache and ache in the bladder are experienced, along with dark urine, containing deposits of uric acid and blood.

Increased urge for urination, but a very small amount of urine is passed at once.Burning sensation while passing urine is indicated along with swelling of the urethra.

•BERBERIS VULGARIS 

Renal colic < left side. Stitching, cutting pain from left kidney following course of ureter into bladder & urethra. Burning & soreness in region of kidneys. Pain in small of back, very sensitive to touch in renal region <when sitting & lying, from jar, fatigue. Numbness, stiffness & lameness with painful pressure in renal & lumbar regions. •CANTHARIS 

Constant urging to urinate, passing but a few drops at a time, which is mixed with blood. Intolerable urging before, during & after urination. Violent paroxysms of cutting & burning in whole renal region. Violent tenesmus & strangury. Urine scalds him & is passed drop by drop. Membranous scales looking like bran in water. Urine jelly like, shredy. Pain raw, sore, burning in every part, internally & externally. Over sensitiveness of all parts. Drinking even small quantities of water increases pain in bladder.

•CALCAREA RENALIS

It is a specific remedy for renal stones.Helps to break tendency to form stones. 

•CHAMOMILLA

Insupportable pains in loins & hips. Pains seem unendurable, drives to despair < by heat, evening before midnight, with heat, thirst & fainting with numbness of affected parts, eructations <. Violent pains, drive him out of bed, compels to walk about. Over sensitive to pain, to open air. Cannot endure anyone near him, is cross, irritable, easily angered.

•EPIGEA REPENS

Renal calculi, gravel, uric acid deposits. Fine sand in urine of a brown colour. Dysuria, tenesmus after urination. Burning in neck of bladder while urinating. Chronic cystitis, mucopus, pyelitis, incontinence of urine.

HOMOEOPATHY FOR SCIATICA

Homoeopathic medicines are very effective for curing sciatic pain . Some of the important remedies are given below.

COLOCYNTHIS 200—Colocynthis stands the first remedy of choice for sciatic pain.It can work wonders in treating sciatica. It is mostly suited in treating righted sciatica pain. Dul, stitching pains in the hip which come suddenly and shoot down to the posterior part of the thigh or to knee or foot.  Pain is relieved by heat and hard pressure but worse by touch or motions.

CHAMOMILLA 200-Chamomilla is another effective remedy for sciatica of very sensitive patients who feel the pain much more than what it actually is. It is also having a mental symptom of – I would prefer death rather than the pain.There is a numb feeling associated with sciatic pain. The pain is more at night and also by motion.

GNAPHALIUM 30—Gnaphallium is prescribed for sciatica when numbness associated with pain.Numbness alternates with sciatic pain. There is intense pain in the nerve accompanied by cramps or alternating with numbness. The pain is worse from lying and walking and better from sitting. The patient experiences pain in calf and feet.

RHUS TOX 200—Rhus tox is an effective remedy for sciatica due to exposure to moist cold weather or from muscular exertion. The pain is more on the right side. The sciatic pain is more in bed at night or when at rest. First movement gives terrific pain which is relieved after walking a little

MAGNESIUM PHOS 1000—Magnesium phos is prescribed when the patient experiences lightening like pain which are better by warmth. The pain comes and goes rapidly. Magnesia phos is considered to be a right sided sciatic remedy.

ARSENIC ALBUM 200- Arsenic alb is effective for sciatica when the patient has been subjected to  repeated attacks of malaria and the pain is intermittent. There is violent, drawing , burning, and tearing pain in  left hip extending to thighs. There is great restlessness and is better from hot application.

GINSENG 30-Ginseng is considered to be a specific for sciatica, when there is frequent desire for micturition. There is a feeling of contraction ad stiffness in the affected part.

GUAIACUM OFF. 1000-Guaiacum is tried for sciatica when well selected remedies fail. There is gouty , tearing , lancinating pains and feeling of contraction in the affected limb. It has acidity, coated tongue, filthy taste and breath.

AMMONIUM MUR. 30-Ammonium mur is effective for sciatica of left hip as if the tendons were too short. Legs feel contracted, limps when walking . Painful jerks, feet feel as if asleep. Pain worse while sitting but better on lying down.

BELLADONNA 200-Belladonna is prescribed when there is sudden violent pain and redness and heat of the affected parts. Here the pains are like jerks or shocks.The lower part of back feels as if broken.There is a full sensation in buttocks and thighs.There is restlessness and dry mouth and throat. The sciatic apin is worse fro heat, checked sweat, jarring, pressure , hanging down the leg , better from bending backward or by bed rest.

*Cimicifuga racemosa*

Pain in the ovarian region; shoots upward and down the anterior surface of thighs. Pain immediately before menses. Menses profuse, dark, coagulated, offensive with backache (menorrhagia, dysmenorrhea), nervousness; always irregular. Ovarian neuralgia (ovaritis, cyst). Pain across the pelvis, from hip to hip. After-pains with great sensitiveness and intolerance to pain. Infra-mammary pains, worse left side.

*Hamamelis virginiana*

Ovarian congestion and neuralgia; feels very sore. Vicarious menstruation. Uterine hemorrhage, bearing down pain in the back. Menses dark, profuse, with soreness in the abdomen (dysmenorrhea). Metrorrhagia, occurring midway between menstrual periods. Intermenstrual pain. Vagina very tender (vaginitis). Profuse leucorrhea. Pruritus vulva.

*Kalium muriaticum*

Menses, too late or suppressed, checked or too carly, excessive discharge; menorrhagia, dark clotted, or tough, black blood, like tar. Leucorrhea; discharge of milky white mucus, thick, non-irritating, bland. Morning sickness, with vomiting of white phlegm.

*Millefolium*

Menses early, profuse, protracted (menorrhagia). Hemorrhage from the uterus; bright red, fluid. Painful varices during pregnancy.

*Secale cornutum*

SCABIES 

      Human scabies is a contagious skin infestation occurs due to the infestation of skin by the mite called Sarcoptes  scabiei , is characterized by the intense itching that aggravates mostly at night.

CAUSE 

1) infestation with Sarcoptes scabiei  is the main cause.

2)people with weak immune system r more at risk  to have it.

3) people who are living in congested or overpopulated areas have more chances to such infestation.

4)by living in areas with a lack of access to water.

5) prolonged physical contact with an infested person

6) health care workers may develop this while taking care of infested pt

7)by sharing of clothes, towels with infested pt.

SIGNS AND SYMPTOMS

      As we know it is an infestation not an infection, caused by female mite Sarcoptes 

Scabiei that makes burrows in to the skin to live and lays eggs, pt develops symptoms as a result of allergic reaction to the mite and that takes around 2-6 wks time after infestation.

Most common symptoms ras follows-

1)intense itching mostly aggravated by warmth and at night.

2) pimple like rash-

          Pink raised bumps with clear top filled with fluid.

3) burrows of scabiei especially in areas of finger web, feet, wrist, and in folds of skin.

4)crusted scabies in pt with weak immune system as their bodies become the fertile breeding ground for the scabiei.

5) scabies vesicles may develop secondary infection due to repeated scratching.

Diagnostic signs

6) scabiei burrows can be detected by rubbing the infested areas with topical tetracycline solution, wiped with an alcohol wipes, if characteristics zigzag or S pattern of burrow appear after cleaning then pt is infested with scabies.

7) from the infested area a small sample of skin with scalpel is taken and examined microscopically for mites and it's eggs.

PREVENTION, MANAGEMENT AND

T/T-

1) by spreading awareness and promoting mass T/T program in areas from where many cases of scabies have been reported.

2)along with the pt , all those who come in his/ her contact should be given t/t even if they are asymptomatic.

3) proper cleaning of pt surrounding since mite can survive for 2-3 days without a host.

4) healthy diet full of green vegetables and fresh fruits for healthy skin.

5)by avoiding coming in contact with infested pt and his belongings.

6) scabies pt should be kept isolated to prevent spread of scabies

7) health care workers who r taking care of infested pt must be provided with protective garments like gown, gloves and shoes cover 

8) washing beddings, towels, clothes of pt with hot water and machine dry.

9) Homoeopathic medicines show excellent results in scabies, some of imp medicines r as-
